Underlining of a number may indicate:

When a publisher’s name is clearly false and it is certain who the real publisher is, then only the latter name is listed in the register. E.g. 382.
This also applies when the title page mentions a false place name and it is certain who the publisher is. E.g. 472, 975, 1024.
A question mark (?) after a number indicates: doubtful or pirate edition: 37?, 209?, 750?, 936? 1113?
Regarding fictitious names. See Pratanus; here we know the real publisher. See Schouten, Nicolas, 914; here we don’t know the real publisher. Neither do we with 987; compare with Pierre Marteau.
Books that don’t mention publisher or printer, but mention one or more place names are listed in the register with that/those place name(s), underlined when there is more than one.
When a book appeared without a publisher’s name, but with a place name, and there is an educated guess -no certainty- who the publisher was, then the book number is listed with that place name (without question mark) and also with the name of that publisher (with question mark). E.g. 995? and Amsterdam.
When an imprint only mentions a place name and it is certain that the book appeared in another known place, than only the latter is mentioned. E.g. 262 (:Amsterdam), 643 (:Utrecht).

Cooperating family members are mentioned as a combination, e.g. Hendrik Boom & de Weduwe van (the widow of) Dirk Boom; all publications by Johannes & Gillis Janssonius van Waesberge. In these cases the numbers are underlined. No underlining occurs when members of a family invariably work together and so form one entity. E.g. John & Awnsham Churchill.
